00:50Pero, Doc, we want to know, ano po ba itong postpartum depression?
00:53And, syempre, may mga tinatawagin tayong baby blues sa pagkapanganak.
00:59Tell us more, what's the difference between the two also?
01:01Oo. Well, you have to understand, no?
01:05There are a lot of changes sa katawan ng babae kapag nanganak.
01:09Definitely, yung mga hormonal changes, nagpa-fluctuate lahat ng mga hormones na yan.
01:14Okay. So, that may cause, like, parang mga postpartum blues.
01:19Postpartum blues may be a normal occurrence sa mga pregnant.
01:23I remember when I was pregnant, and I just gave birth, bigla na lang parang, wala, you know,
01:28bigla ka parang medyo maiiyak na na lang, awang-awak ka sa lahat ng tao, you know.
01:32That was normal, and that lasted for like maybe a few minutes.
01:35But usually, postpartum blues will affect like mga 50 to 80 percent of women who gave birth.
01:41But then, after that, mawawala siya ka.
01:42Regardless, kung first or second child, or pang ilan child, pwede maranasan?
01:46Pwede, pwede. Kasi nga, nagpa-fluctuate yung mga hormones, and that will affect the mood.
01:51So, that could be a normal occurrence, you know, you just like sleep properly, you know, you get the help that you need.
01:57Pero, mawawala na lang siya on its own. So, you don't need treatment for that.
02:01But you need a lot of support, no?
02:03Pero you can also have what we call yung parang postpartum depression.
02:07That one is a little bit less common, mga 10 to 15 percent of women who gave birth.
02:13Yun kasi, medyo mas grabe na the functioning is being affected already.
02:18So, ibig sabihin yan, eh, yung mga kailangang gawin, minsan parang ayaw nang gawin,
02:22hindi na minsan maalagaan ang bata, you know, sobrang feeling nila parang, you know,
02:26parang down na down ang energy.
02:29So, kapag ganoon, that's postpartum depression, talagang kailangang gamutin na yan.
02:33So, that's the difference.
02:35Okay.
02:36Okay.
02:36Okay, ma'am, Doc, nabanggit niyo yung mga, yung isa sa mga cause, yung hormones nga po.
02:40Pero ano yung mga maaring risk factors nga po dito po sa postpartum depression?
02:45Depression.
02:46Ah, okay.
02:46That's a very good, ano.
02:48So, the hormonal changes, definitely, that's a biological part of it.
02:51Okay.
02:52Pero yung mga risk factors mo, the highest risk factor will be previous history.
02:56Kunwari, kapag may previous history na, let's say, of depression, of anxiety,
03:03or may pagsabihin mo, ito sa difficult pregnancy, or wala kang masyadong support,
03:08or maybe ito sa unplanned pregnancy.
03:10So, all of those pwedeng mag-contribute to having like postpartum depression na symptoms.
03:15Okay, what could be the symptoms here kapag nakaranas ng postpartum depression?
03:20Kasi, usually, the stigma here is that, ako, parang di naman yata totoong may problema yung nanay ko,
03:27kaya itong kapatid ko, pagkat ng anak.
03:30Tumaga, parang, I think we have to break the stigma.
03:32So, we wanna know what are the symptoms?
03:34Yeah, you know, and I'm glad that you brought that up.
03:36Kasi nga, parang may stigma talaga, psychiatric illnesses, unless ikaw yung makaramdam talaga.
03:41Isipin mo, nag-inerte lang to.
03:43You know, tatagan mo lang, mag-break ka lang.
03:45You know, it's not like that.
03:46You know, it's really an illness.
03:48And the symptoms will be, parang siyang major depressive disorder with postpartum onset.
03:54So, usual symptoms niyan will be depressed mood, definitely.
03:58Loss of interest in things.
04:00Poor appetite.
04:02It's either you eat too much or you don't have appetite.
04:04Ayaw mong kumain.
04:05Pwede rin, either hindi ka makatulog or tulog ng tulog.
04:09Walang masyadong energy, parang pagod na pagod.
04:12Feelings of hopelessness, helplessness, wala ng pag-asa.
04:15Or parang inappropriate guilt, lahat na lang, kasalanan ko, dapat ganun, dapat ganun, you know.
04:21And, um, syempre, ang worst of it is pag nagkakaroon na ng mga thoughts of dying or mga suicidal thoughts.

04:44Ito, ito, kunwari, kapag hindi pa nakakaintindi, wala kang pang tulong na nakukuha from the people around you.
04:50Yun nga, nabanggit ninyo, Doc, at ni Prof. K, dapat nga mabreak po yung stigma patungkol dito.
04:55So, paano naman po natin matutulungan yung isang ina nga po na nakakaranas po, itong postpartum depression,
05:01at ano po yung mga hakbang na maaring gawin upang maiwasan ito?
05:05Oo. Well, first, the most important thing really will be recognition.
05:10Okay.
05:11You know, dapat naiintindihan, there's really such a thing as postpartum depression,
05:16at kapag na-recognize na yun, then kailangan na nini-encourage to get help.
05:21Okay? So, that's the most important thing.
05:23But other things that you can do, maliliit na bagay eh.
05:26You know, bigyan ninyo ng time para matulog.
05:29I-deck ninyo yung mga asawa.
05:30Yes.

05:37So, dapat yung husband tumutulong, tapos parang makatulog naman si mommy,

05:45Tapos yung mga ano, pwedeng tulungan ng mga kamag-anak.
05:48You know, maganda sa Philippines kasi, maganda yung support system.
05:52So, nandiyan yung mga byanan, yung mga nanay, kapatid, pinsan.

05:56So, and then don't be afraid to ask for help.
05:59Kapag kailangan yung tulong, nahihirapan kayo,
06:01then humili kayo ng tulong.
06:03You know?
06:04So, that's really very important.
06:05Dok, ano po mga paraan o treatment options?
06:09Kailang dapat lumapit sa doktor for professional help?
06:12Ah.
06:12Well, treatment options, that would depend on how bad the depressive symptoms are.

06:31You know, may evidence-based na parang from a mild to moderate exercise might work.
06:35Like mga meditation, all of those things might help.

06:40But, kunwari, kapag talagang, ano na, medyo mas grabe na mga symptoms, then you might need to take medications.
06:47Ayaw.
06:48Oh, doctor.
06:49Yung mga medications, siyempre, ang concern yan will be, you know, ay paano yan kung nagbe-breastfeed ako, parang ganoon.
06:55Then you will have to work very closely with the pediatrician, you know.
06:59So, kunwari, kapag talaga may symptoms, sometimes you have to put the mother on antidepressants even before mga anak.
