Après un sprint massif lors de la 4e étape mardi, cette 5e journée offrait un parcours trop exigeant pour les purs sprinteurs de ce Tour d'Italie. Avec un final accidenté, ce sont des sprinteurs-puncheurs qui se sont disputé la victoire à Matera. Après une étape mouvementée, marquée notamment par une accélération de Primoz Roglic (Red Bull-BORA-hansgrohe), c’est finalement au sprint que Mads Pedersen (Lidl-Trek) a décroché une troisième victoire. Le Danois a résisté de justesse au retour de Edoardo Zambanini (Bahrain Victorious). Tom Pidcock (Q36.5 Cycling Team) complète le podium.
