00:00It's a Women's Month, Women's Health Reel, and today we will talk about Saneidoc.
00:10All women have the right to decide when they want and don't want to get pregnant.
00:16All women have the right to do family planning and use contraceptive or birth control methods.
00:23But the question is, what are the side effects of using these methods?
00:30To give us a free telemedicine, we have OB-GYN Dr. Anafe.
00:37For because of Gloria.
00:39Doctor, good morning. Welcome to Rise and Shine Philippines. This is Prof. Fie together with Meiji.
00:45Good morning to you, Professor Fie and Ms. Meiji.
00:51Good morning, Dr. Anafe. You look beautiful in the morning.
00:55Alright, Doc. What are the types of contraceptives and what are the most common ones that are being used now?
01:05Okay. Maybe we need to categorize the contraceptive pills or contraception for that matter into long-acting contraceptive.
01:17Meaning, the long-term use.
01:20The intrauterine device or the small metal with a little plastic that is placed inside the uterus.
01:31So that, it can last for 5 to 10 years.
01:35Aside from that, another long-acting is the implant.
01:40The implant or the next implant is placed inside the arm and it can last for 3 years.
01:49So after the long-acting contraception, we also have the so-called permanent or irreversible.
01:57Like the bilateral tubal ligation.
02:00That is when the female's tubes are tied and when it's a male, it's vasectomy.
02:07And then, we have the most reversible, the oral contraceptive pills.
02:14That is what we drink every day to provide contraception.
02:20So the hormonal birth control pills, I mean hormonal birth contraception, it can be pills or it can be injectable.
02:30And sometimes, the intrauterine devices also have hormones, even the implants.
02:36And we have the so-called barrier method, which is the condom.
02:41That is what is used only when necessary during sexual intercourse.
02:46The most beneficial use of condom is protection against STD.
02:51And then, there are natural forms of contraception, which is one, the withdrawal.
02:57Second is the fertility awareness, that is by the calendar method.
03:03And the breastfeeding or lactation amenorrhea method.
03:08There, there are a lot of contraceptives.
03:10Go ahead, Doc. Go ahead. Sorry.
03:13Yes.
03:14So for all of these contraceptives, the most popular is the combined oral contraceptive pills.
03:22There. Yes, she also mentioned that in the Philippine Statistics Authority or PSA.
03:26And I remember, my mom is a midwife, that's why I'm very exposed to these contraceptives.
03:30I can really see the condoms, the pills.
03:35And these women are saying,
03:40Oh, my wife is like this, like this, like this.
03:43But Doc, all contraceptives have side effects, right?
03:49So can we know what are the side effects of contraceptives?
03:54Maybe we should start with the pills, IUD, contraceptive injections, condoms.
03:59Please go ahead, Doc.
04:01So the combined oral contraceptive pills are the most popular,
04:06mainly because it's very accessible.
04:10Sometimes, it's free in health centers.
04:14And then it's easy to use.
04:16You just drink it every day.
04:18And then it's reversible.
04:20Meaning, when you stop it, it loses its effectivity and it's gone from your body.
04:28So it's very easy to use, reversible, and very much accessible.
04:33That's the benefit with using the combined oral contraceptive pills.
04:38And it's also not very expensive.
04:41The price is just right.
04:44Sometimes, less than 100 pesos or around 50 something,
04:48you can buy combined oral contraceptive pills.
04:51So what are the common side effects with using the combined oral contraceptive pills?
04:57So for less than 10% of the patients, they can experience breast pain.
05:05It's like your breasts are heavy.
05:07Bloatedness or the feeling that you're fat.
05:11And then nausea, meaning you feel like you're going to puke.
05:16Or sometimes, your head hurts.
05:19But these side effects would usually go away after three months of use or even less.
05:26The rare side effects, the so-called venous thromboembolism,
05:31which is when blood clots start to show up,
05:34can be encountered in less than 0.1% of those using the pills.
05:42So maybe the most common concern of our countrymen who want to use the pills
05:50is that they think they're going to gain weight with the use of the pills.
05:55That's what they usually ask.
05:57Actually, based on studies,
06:01the weight gain with the use of the pills is very minimal.
06:07Usually, the weight gain is because we're getting older,
06:11our metabolism slows down, or it really depends on our lifestyle.
06:17So another concern of our countrymen who are always asked is
06:22that they feel dizzy when they're using the pills.
06:26It's like their heads are spinning.
06:28Actually, maybe we can attribute the mood changes to patients
06:33who really feel dizzy even before taking the pills.
06:37So it doesn't really affect the moods of a large percentage of patients.
06:47Then there's libido.
06:49It's said that drinking pills lowers libido.
06:55That's also very controversial.
06:58No one says that drinking pills really affects libido.
07:05And some of our countrymen are afraid that when they drink,
07:10they won't feel dizzy later on.
07:13Actually, when you stop taking the pills,
07:17your baseline fertility will return instantly.
07:21So it's not true that you won't feel dizzy when you take the pills.
07:27And it's not a lifelong commitment when you decide that you want to take the pills.
07:32Because at any point when you decide that you don't want to take the pills,
07:37you can already stop.
07:39That's the benefit of taking the combined oral contraceptive pill.
07:44So, do you have any questions about the side effects?
07:48Yes, I can say that it's hard to be a woman because of all the side effects.
07:54But you mentioned that it's like a myth, right?
07:57The low libido, the weight gain, the lack of sex, the mood.
08:03There are no proven studies that show that it affects a large percentage of people.
08:09But the other contraceptive pill, I believe it's called IUD, right?
08:14Is that right?
08:15It's the one that you don't have.
08:17Is that right, doc?
08:19And also, I want to ask,
08:21I think you're talking about IUD.
08:23Yes, IUD.
08:24It's the one that you take for three years.
08:27Is that right?
08:28And you also mentioned vasectomy.
08:31I'm just curious, doc,
08:33is it right for a man to do that?
08:37Is it reversible?
08:41Actually, that's why we call it permanent,
08:44the vasectomy and the bilateral tubal ligation.
08:48It's basically,
08:51it's very hard to get fertility back,
08:55or to get back the cut tubes,
08:57when it's already cut.
08:59It's only a very small percentage of success
09:05when it's done.
09:08The vasectomy and the bilateral tubal ligation,
09:12less than 30%,
09:15that's the only way to get fertility back.
09:19So, okay, let's try to answer your question one by one.
09:24So, let's say, or let's talk about the benefit
09:29of using the combined oral contraceptive pills
09:32before we keep on saying the side effects.
09:35First, contraception is really good,
09:38very effective pills in providing you contraception.
09:43Second, when you're on pills,
09:46your menstrual blood weakens.
09:49The pain of your heart also goes away when you're on pills.
09:53And then for the others,
09:55for other kinds of contraceptive pills,
09:57it also removes pimples,
10:00and the hair,
10:02or the excess hair on the body of women.
10:06And it also has almost 40% prevention
10:11of ovarian cancer and endometrial cancer,
10:14and 20% for colon cancer.
10:18So, there are also many non-contraceptive benefits.
10:21The use of combined oral contraceptive pills.
10:26So, the injection,
10:29the injection is also very popular.
10:32It's often given or administered in health centers.
10:36It's a Depo-Provera.
10:39It's a hormonal contraceptive pill also.
10:42It's injected every 12 to 13 weeks.
10:47In the first use,
10:49it can cause irregular menstruation.
10:52It can also cause a slight weight gain.
10:56And it can also cause a slight acne.
10:59But the good thing about it is,
11:01you don't need to drink at night,
11:03and every three months,
11:05you just need to go to a healthcare worker
11:08to give you contraception,
11:11and it doesn't have estrogen.
11:13So, it has a lesser side effect
11:16in terms of our patients
11:19who cannot take estrogen.
11:22Alright, Doc.
11:23On that note,
11:24thank you very much for sharing your knowledge with us.
11:27Again, we are joined by Dr. Anafe Orbegos of Gloria.
11:30Again, thank you, Doc, and stay safe.
11:32Thank you, Doc.
