00:00In the first instance, the Philippine Heart Association officially announced the Heart Failure Awareness Week.
00:09Its purpose is to raise awareness about heart failure and the effect of early detection and its management.
00:18And aside from that, Audrey, we will now be joined by the immediate past president of the Philippine Heart Association, Dr. Ronald E. Cuyco.
00:28Good morning, Doc, and welcome to RSP.

00:32Good morning. Thank you for inviting us. Of course, thank you, Noel and Audrey.
00:37And of course, to our viewers of this program.
00:42And of course, we will talk about important heart diseases.
00:47Well, this is one of the reasons why Filipinos are dying.
00:54But Doc, for our viewers to understand, as a beginner, what is heart failure?
01:01So, this heart failure is actually a condition of the heart, a heart disease,
01:07where the ability of the heart to supply blood to different parts of the body, such as the brain, kidney,
01:17is limited or compromised because of this problem, such as a heart attack.
01:23So, there are different reasons for this.
01:25In other words, this heart failure is at the end of different types of heart diseases.
01:30For example, there is a blood clot in the heart.
01:32This is what we call ischemia.
01:34If there is an attack in the heart, this causes a weak heart attack.
01:42Another reason for this problem is the valve.
01:45In the Philippines, this is what we call as rheumatic heart disease.
01:49And another one is the muscle itself.
01:52The muscle of the heart sometimes weakens suddenly because of an infection.
01:57This is common during COVID.
01:59This is common when a patient dies because the muscle weakens and there is an infection.
02:05We call it myocarditis.
02:07Because of this disease, what are the symptoms of heart failure
02:12and how can you differentiate the symptoms of heart failure compared to other heart diseases?
02:19So, it is very important that we mention the symptoms one by one.
02:24What does a patient feel when he has a heart failure?
02:28First of all, of course, it is easy to get tired.
02:31If you used to walk a long distance or you can climb three flights of stairs,
02:38now, you can only do it up to the first floor.
02:40This means that something is happening.
02:42The oxygen in your whole body is running out.
02:46The other thing is towards the late stage of heart failure,
02:50you can see that the heart is getting bigger.
02:54And sometimes, it is hard for them to sleep.
02:56It is different, right?
02:58It means that if it grows because of heart failure, it is because there is water.
03:03So, the heart can't pump and it can't circulate well.
03:08So, other patients have water in their lungs.
03:14So, when they lie down, they can't lie down flat.
03:17So, they would prefer to stand up.
03:19And the other thing that is dangerous is that they can only sit when they sleep.
03:23So, most of the time, they can't sleep well.
03:26Their food intake is low.
03:27Their body is lacking in nutrients because their intake is low.
03:34Others have chest pain.
03:37So, it is hard to differentiate the symptoms of heart failure from other types of heart disease.
03:43Because it is part of the continuum of heart disease.
03:47So, if there is a heart-related disease or heart valve disease,
03:51towards the end, they will present as heart failure.
03:55So, it is actually the same.
03:57So, the message that we want to convey to the countrymen is that there should be early detection.
04:05If you feel breathless and it is new to you,
04:08so, if you see your heart beating or palpitations,
04:12consult immediately.
04:14Don't be consistent.
04:15Because sometimes, it is because of fatigue or any other reason.
04:21But if you consistently feel in your heart that it is tightening,
04:26that is what you should bring to the doctor.

04:29That is right, Audrey.
04:30We also need to observe ourselves.
04:32But if you have risk factors like diabetes, high blood pressure,
04:38or severe pancreatitis,
04:40then you should consult.
04:42But if you are in a situation where there is stress,
04:47like if you wake up early in the morning,
04:50sometimes, there is a little bit of fatigue.
04:53But if it goes back,
04:55if there is joint pain in the chest,
04:57if you feel breathless,
04:58I think you should consult a doctor.
05:00I think it is also related to the family history.
05:03Yes.
05:04It is like a genetic predisposition.
05:09When it comes to treatment,
05:11how is the treatment to control the symptoms of heart failure?
05:16Most patients are fluid overloaded.
05:20There is water in the body.
05:22As we said, there is water in the lungs because the pressure is increasing in the heart.
05:26The lungs are connected to the heart.
05:28Sometimes, there is a fever.
05:30So, what we give is to make the heart beat faster.
05:33And then, their heart rate increases faster.
05:35So, we control it.
05:37These medicines can relieve it.
05:39But of course, we have to emphasize,
05:41what is the cause of heart failure?
05:45So, you have to treat it.
05:47For example, if there is a bullet in the heart,
05:49fix the bullet.
05:50If there is a problem with the valve,
05:52it is limited.
05:53Fix the valve.
05:54It is a limited operation.
05:55It is connected.
05:56Yes, it is connected.
05:57So, overall,
05:59what we want to emphasize is that
06:01we should not reach that situation.
06:03We should address the risk factors of this disease,
06:07such as high blood pressure, diabetes, paninigerilio,
06:11all of them, so that we do not reach that situation.
06:13Prevention is better than cure.

06:18the Heart Failure Awareness Week,
06:20are there programs and activities
06:22being prepared by the Philippine Heart Association?
06:24Of course, this is February.
06:26This is Heart Month.
06:28So, in line with the celebration of Heart Month,
06:30one of our focuses
06:32at the Philippine Heart Association
06:34is heart failure.
06:37The prevalence rate is increasing.
06:39In the Philippines, it is about 1.6% years ago.
06:42Even in our neighboring countries,
06:44the prevalence rate is really low.
06:46So, one of the things we gave to the Philippine Heart Association
06:48is that we re-launched
06:50and gave life to
06:52what we call the National Heart Failure Network.
06:55This is brought together
06:57by different medical societies
06:59in partnership with,
07:01of course, the government,
07:03the private sector.
07:05This is brought together with the primary intention
07:07to spread,
07:09to explain to people
07:11what heart failure is
07:13and what are the medicines
07:15and treatment options for it.
07:17First and foremost,
07:19how to avoid it.
