00:00Sunlight, it burns
00:04Shining brightly
00:08Turning the page
00:12Like a new leaf
00:16It never fades
00:20With all the beauty
00:23The brainwaves that surged were associated with dreaming, information processing, and memory recall.
00:29Push another epi?
00:30Please, Jackie.
00:34Still nothing.
00:36Someone get me a TEE right now.
00:41It confirms what we often hear when we revive a patient in cardiac arrest.
00:46That right before their heart stopped, they saw their life flash before their eyes.
00:50I think we've been through.
00:52Thought this was impossible.
00:54Me too.
00:55Not that I remember it all.
00:59I have to go.
01:00Can you stay?
01:01I'll look for you as soon as I can.
01:02I have nowhere else to be.
01:04I have nowhere else to be.
01:05I have nowhere else to be.
01:06I have nowhere else to be.
01:07I have nowhere else to be.
01:08You
01:09I have nowhere else to be.
01:10I have nowhere else to be.
01:11to go. Can you stay? I'll look for you as soon as I can. I have nowhere else to be.
01:30Bailey, have you seen Yasuda Lepacer 10 minutes ago? Nothing. It's because she doesn't work
01:37here anymore. She quit the program. What happened? Just being back here where her sister died.
01:43It was too hard. That's unthinkable. I wish there was more I could have done to help her.
01:49I mean, she's tough. She'll find a way. I'm not so sure about the others. Millen
01:54isn't handling it very well. First year residency. Your fellow interns are all you got.
01:59I'll keep an eye on her. Thank you.
02:01Yeah. We were close to trauma.
02:08Bring it in. Bring it in this way. All right, hold up right there. Watch yourself. Watch
02:12yourself. Watch your backs, folks. Watch your backs. Make room for the emergency beings.
02:16What the hell happened? Warren left us open to trauma, so you're looking at the consequences.
02:21I was going to set up an additional triage tent, and then I was going to come up with some workarounds.
02:24I was chief of staff at the Army Medical Command. This is not a workaround. This is a fireable
02:29offense, so if you get the urge to make another decision without consulting me, don't. This
02:35is Altman. Okay, go ahead. What did you do? Not the time. Ben! All right, that's the last
02:39one. We are closing to trauma. Adam! I'm okay. It's not all mine. Joe Wilson, 38-year-old
02:48female. Wilson? Joe? I'm going to have to call you back. What? Got a gunshot wound over here.
02:53Yeah, his name is Damien, 28-year-old male. GSW to the right chest. No exit. We're going to
02:57intubated on the scene. He was with you? He held up the store. All right, I'll go. Stay
03:02with Wilson. Let's get her inside. She's bleeding. You were shot? No. I'm pregnant. I got you.
03:22I really appreciate you taking me to my hotel. I know they need you at the hospital.
03:26Well, Teddy hired a bunch of outside doctors to pitch in. She has plenty of help.
03:32I can't believe you still drive a truck. What was it that you used to drive? An old Silverado?
03:37I just remember it took up two parking chases. Yeah, and it didn't go over 45 miles an hour,
03:42and only one of the handles worked. It was not what you would call a chick magnet.
03:46You could have driven an RV, and I would have thought it was the sexiest truck on earth.
03:49You hated me. I was madly in love with you. I thought you knew. Hold on. Okay, you were mean
03:59to me. You were my best friend's older brother, and I was a teenage girl who didn't know how
04:04to tell a boy I liked him. What? Wow. Are you okay? Yeah, I'm fine. It's just, um, it's pain where I had the surgery.
04:14It happens sometimes. You had the surgery weeks ago. I'm running you back to the hospital. No, no,
04:18that's not necessary. Any amount of pain could indicate a complication. We're not taking any chances.
04:22beautiful day, isn't it? It's 108 degrees. Sun is out. Everyone's working together towards a common goal.
04:33Mika, quit. What? Why?
04:39Well, her sister died here, and she almost died, and I think she wasn't into the weather, but that's just an observation.
04:46You okay? You two are close. I didn't come here to make lifelong friends, did you?
04:50No. People come and go, and I still have a job to do.
05:02Do you see anything? What's going on? Why won't her heart start?
05:06Right there. The aortic root aneurysm dissected and extended down, occluding the left coronary artery.
05:12The block you're just keeping her heart from starting? It's the tear. It caused a massive infarction.
05:16We need to revascularize her coronary artery to restore blood flow, then put in an aortic graft.
05:20Let's prep for a stenotomy. I know you wanted to take care of this first, but I maintain that this was the right call she could have dissected, no matter what.
05:26At least she did it in the safest place where we can treat it. An aortic dissection is never safe.
05:31All right, Jackie's already been under for hours. We need to move faster. Let's go.
05:33Dr. Shepard. Whatever it is, it's got to wait. It's Dr. Adams. He wants you to know he's okay.
05:38Do us up to speed. Single GSW to the right chest. No exit wound. He's hypoxic. He has a right hemothorax.
05:48His fast exam on his heart was negative. We need to prep for a chest tube.
05:53Okay, bullet fragments are localized on the right chest. Let's order a type and cross and trauma panel.
05:58Can I do the chest tube? Get her sterile gloves. His sats are dropping, so we need to move quickly.
06:02I bet when he walked out of the house with a gun today, he didn't expect to be the one getting shot. Ready?
06:07Mm-hmm. All right, you're gonna make an incision between the fourth and fifth rib space.
06:14Now you're gonna pop through the pleura.
06:17Mm-hmm. Good.
06:19And now dilate by opening the clamp. Time for the tube.
06:23Yeah, keep going.
06:25All right, he's putting out a lot of blood. We need to call up to the OR.
06:32Nice work, Mel.
06:37BP's 105 over 65.
06:39That's good. Uh, how many weeks are you?
06:4216 weeks. I, uh, had an appointment a week ago and everything looks fine.
06:46Have you paid Joby? I can do it.
06:48No, you will not. You will get yourself checked. Dr. Warren?
06:52Come on, you heard her. Let's go.
06:53No, I was just grazed. It can wait.
06:55Do you think your colleague wants you to see me do her ultrasound?
06:58No.
06:59Go. Now. Don't let him out of your sight.
07:01Wait, wait, wait, Adams.
07:04Okay, come on. Let's go.
07:07Bailey, I need to see for myself.
07:09You are not your doctor, okay? We paged OB. In the meantime, hmm.
07:15All right, the placenta looks great. Ain't no abruption.
07:22And where is the...
07:24We have a strong heartbeat right there.
07:28Hmm.
07:30What about the other one?
07:32There's an other one?
07:34Okay, let me see.
07:37Oh, there it is. Oh, beautiful.
07:44It's just these two, right?
07:47As far as I'm aware. Yeah.
07:50You okay?
07:52Mm-hmm.
07:53Nope.
07:54Okay.
07:55All right, someone help me get her the OB.
07:58And where's Lincoln?
08:00All right, hold on.
08:02Thank you, thank you.
08:04We were finishing a surgery.
08:08Are you okay?
08:09Yeah, it's just superficial abrasions,
08:11sterile strips, and I'm good.
08:12Yeah, and I've got sight coming down for an eval.
08:14Yeah, I don't need that.
08:15Yeah, a whole hospital policy.
08:16You have to be cleared before you can go back to work.
08:18I know it's important. You've got to process.
08:19Have you called your mom? Do you want me to call her?
08:21Well, I really do not.
08:22Oh, they just extubated Jackie.
08:23Go check on your patient, all right?
08:25There's already too many people standing around this bed.
08:28All right, you stay with him.
08:30Quan can cover post-op.
08:32Okay.
08:37And, uh, I also have to go check on something.
08:48What happened?
08:49Wilson and I went to the convenience store to get ice.
08:52A guy tried to rob it.
08:53I got lucky.
08:54He was on his way up to the OR.
08:57Thank God you're okay.
08:59Griffith, I need you to help Millen transport this patient up to the OR.
09:08That's him.
09:10That's...
09:11No, I can't, no.
09:12Yeah, yeah, you can. He's just another patient.
09:14Except he tried to shoot.
09:15But he didn't.
09:16Just another patient.
09:18I have to do a consult, have him prep by the time I get up there.
09:24Chief, I'm on a Dugu service.
09:26It's all hands on deck.
09:27And if he's got a problem with that, frankly, I don't care.
09:31So, Dr. Marcus is on his way.
09:41No.
09:42Absolutely not.
09:43Literally anybody else.
09:44Uh, well, DeLuca is in Italy.
09:46Patel's on leave.
09:47Kincaid's in the OR.
09:48idly patel's on leave kinkade's in the o.r isn't um dr marcus the chief ob resident it doesn't
09:54matter he's not taking a trip down there okay i know it can be difficult to be the patient
10:00with someone you usually work with but you need somebody in god i just heard what happened are
10:07you okay yeah marcus yes i finished med school undergrad only took me two years it's bailey
10:14right wow twins it's a party so based on the initial workup it's likely stress that's causing
10:22the bleeding okay okay that's good that's good she's also having acute back pain wilson next steps
10:30kidding uh pain and bleeding could also indicate other complications so i'd like to okay i was just
10:35getting the kids settled i came as fast as i could it's okay okay what about the bleeding it's slowed
10:42down but i'm feeling heaviness below i'd like to do a pelvic ultrasound i'm assuming you're dead
10:51so let's give them a minute
10:57no you're right there do you need a minute or my son is throwing a tantrum and my ex wants me to
11:03calm him down tell me you have good news well your ekg is normal but your white blood cell count is
11:09elevated which means i know what that means last time i needed surgery is that what's happening and
11:14do i have to go through this all over again could just be a mild infection or even stress or dehydration
11:18let's not worry until there's something to worry about okay hey what's happening uh just some chest
11:24pains and hopefully a disproportionate amount of anxiety her ek she's normal but she has leukocytosis
11:29and she is slightly febrile we're still waiting on blood cultures all right let's admit her start
11:34her on broad spectrum antibiotics get a chest x-ray and ct with contrast is now a good time to worry
11:39it's just it's been such a day you know why don't you go with her i can get cast to come down
11:43and help out it'll help with the scans going much quicker if you're there all right you're back
11:47and have it covered yeah yes no i know no i i do still love you i just i just needed some space and
12:03like a minute away from school i'll i'll call you tomorrow dave
12:09uh yeah he called like eight times i had to pick up i just i didn't know what to say you know i didn't
12:19want him to know that i was here so i just i just tried to pretend like everything was normal it's
12:24fine it's anything but fine i didn't think this through and you're slammed i shouldn't have just
12:30showed up out of nowhere no it's really okay at some point i need to give dave an answer it's not his
12:35fault that this is this is what it is i really need to answer this but i want a chance to talk about
12:44this hang out at my place and we'll talk all about this tonight if you need to go home you'll go home
13:05we'll never understand how a person can point a loaded gun at a stranger well people can hurt
13:10each other in all kinds of ways i'm sure it's killing is suda having to leave you don't sorry
13:18i didn't mean to bring it up when you want to talk about it we will talk about it and i won't be
13:21talking about it with you did i do something i'm just sick of pretending like we all mean something to
13:25each other i get that intern here is hard and that we're all trauma bonded or whatever but we're just
13:31co-workers i don't need you to comfort me or to understand how i feel i can take care of myself
13:39i've been doing it my entire life how's it going in here great
13:44he's about to code all right griffith strap him in melon prep him
13:58you've got the other side of the bell yeah i got it did anybody contact the blood bank to activate
14:02the mtp uh i'm not sure i can check it you are the intern on this case you need to make sure that
14:07you are on top of the orders i'll do it right now all right he can't wait okay we need to scrub in
14:14right now yeah i was able to successfully clip the posterior aneurysm but we ran into a complication
14:23when we tried to wean her off bypass uh so what happened jackie's stable but she had an aortic
14:31dissection which caused a heart attack my 17 year old had a heart attack i opened her chest and i
14:37put in a graft and thankfully she's an athlete so her heart was able to recover from the mi
14:44i i don't understand why do these complications keep happening well we knew her heart was at risk
14:50due to the root aneurysm her brain was a greater risk at the time her post-help labs came in thank you
14:55dr kwan how long does the anesthesia last hey baby
15:05i'm still feeling groggy and my feet your feet and my legs dr kwan would you get me a needle please
15:19all right jackie let me know when you start to feel this
15:26did you start yet
15:31why can't she feel her legs
15:42now there's no written rule on it but people usually stand on the other side
15:46during a pelvic exam to respect the area right sorry i need you to relax wilson okay
15:52you heard about kinkade's uterine sarcoma case poor woman's second reoccurrence she got four kids in
15:59a death sentence you should see the scans they're awful honestly it's like a horror movie puppies
16:06they're adorable and calming i'm thinking about getting one for proof so your cervix is 18 millimeters so an
16:15imminent risk for preterm labor but you're just 16 weeks she's 16 weeks which is why i'd recommend a
16:22cervical cerclage it's placing a suture around the cervix to keep it close he's a doctor oh hospital
16:28babies are you going to name them gray and sloan what are the risks of the cerclage um well i could
16:36get an infection or it could cause damage to my cervix like um scarring or stenosis other options please
16:42this is the option besides terminating the pregnancy cervical cerclage is very safe and it is the absolute
16:48best way to preserve the health of dr wilson and the babies right now but if you'd like i could give
16:53you some time to think about it give me the consent forms
17:05my four-year-old listens better than you do yeah well unless your toddler is also a surgeon during a
17:09record-breaking heat wave i don't think that's a fair comparison you shouldn't be seeing patients i'm
17:13still on a schedule and the er is over capacity okay let me rephrase i can't let you treat patients
17:20right now fine all right i see life and death every single day business as usual all right you know
17:25fine if you want a job to do why don't you head over to the clinic and help out with the cooling center
17:32mrs long in bed six needs more stool softener
17:34do you see anything not yet you championed me what you told sky i could do this but you told me i
17:46could do this now i'm sitting here silently panicking about what i've done to this young
17:50woman and you're hardly saying a word you don't think i'm panicking you're not the only one who
17:53operated on her i should have said no from the second you brought me in i had a bad feeling about
17:58this why didn't you say anything because you prematurely accepted the case you knew it had a
18:01cardiothoracic component to it but you accepted it before getting my opinion on it she was admitted
18:05before i even saw her no no do not put this on me you could have said no right after you walked out
18:11the door maybe you didn't because of how you treated her mother nurse connie said a cold front's coming
18:17in the heat should break soon at least there's that skins are up kwan what do you see
18:24uh head ct is negative for stroke but the cta shows the dissection flap propagating into the
18:33descending aorta which means blood flow to the spinal cord is compromised
18:42i think it's okay is this is the word hey radiology is a little backed up we'll get you in
18:50there as soon as we can you don't have to babysit me remember how annoyed you would get when your
18:53mom would make you watch me and megan when she went to the store but that was a chore this is an excuse
19:00you made that face earlier with teddy what face that i sent some tension
19:06um sorry you don't have to answer that i i think my surgical complications removed my filter or maybe it
19:12was my divorce or i don't know sorry it's fine marriage is marriage is complicated but i can't let
19:18this happen any longer are you trying to go up or down up i'm okay damsel in distress myself yeah
19:26there yeah um there
19:32hi hi
19:39uh radiology's ready for you so i'll go find uh orderly okay okay good idea yeah
19:48why don't we just first start you on progesterone and increase your monitoring why can't you trust
19:54that i'm making the right decision i'm a pregnant obgyn i want a cerclage i'm getting a circle you can't
20:00just wave your specialty in front of my face every time we have a disagreement i've got hundreds of
20:04these i do them all the time but you don't get them all the time and what about marcus how many has he
20:08done shouldn't we be waiting for somebody born before tick tock he's in my year i have t-shirts older than
20:14him link okay are heartbreaks elevated um maybe some deep breaths oh god i wish dr deluca was here
20:20first of all i would never have had marcus's hands inside of me and second of all she would tell you
20:24how safe a cerclage is maybe she could tell you that this isn't just your decision she would never
20:29tell me that are you thinking clearly what you witnessed a shooting today you could have been
20:35seriously hurt okay time out okay you are now the one who is seriously hurting her her bp is 140 over
20:4290 and that's too damn high for her and for me add to that uh 18 millimeter cervix and you are tripling
20:50her chances for contractions take a walk okay i'm sorry it's just i am not asking look at me
20:57in and out good in out i've exposed the damaged tissue i was gonna lose an entire lobe of his
21:11lung due to one bullet gia stapler his bullet that he loaded into his gun we should invite the
21:17lawmakers voting against background checks to assist on these surgeons while violent crime rises along with
21:22the temperature i read a story about a guy who drove his truck into a hardware store during a
21:26cheat way we killed four people well there's some bleeding at the staple line all right 20 pds
21:34thank you bokeh damn it we can't get visualization suction
21:40no all right griffith millen what do we do to the table exactly all right team you heard them
21:45anesthesia when you're ready oh my god why is he moving all right stop stop stop
21:52watch it live oh secure airways alive hey some orderlies let's assess him and get him back on the
21:59table right now gently gently
22:04let's keep him on amble until we get him back on the table where are those orderlies damn it his
22:10central line has come out all right shut off all the lines you got it lap sponge all right keep
22:15pressure on his ij should we pack the thoracotomy no here you go all right millen get me a wet towel
22:22millen all right take this
22:25how the hell did this happen must not have been completely strapped in bill and hold here if your
22:30wife finds out she's gonna fire me all over again keep applying pressure bring in the orderlies
22:35come on fellas we're gonna carefully lift him back on the table everyone set on my count one two three
22:46you want to puncture her spine we placed a small tube in jackie's lower back to drain the excess fluid
22:52around her spinal cord there is a chance that i could reverse jackie's paralysis this morning
22:58you assured us that you could take care of her aneurysms you seemed so certain about it that was
23:03before i saw the larger aneurysm she is my only child when did you know that this was going south
23:09was it when you had to open her skull or when you had to stop her heart or when you noticed there was
23:15something wrong with her spine i mean i i need to know i need to know did you ever think that you
23:20could save my child or was i just a fool for believing it i did think that i could save your
23:25child and she is alive and aneurysm free i do not make false promises because i have seen too many
23:31bad outcomes this is not a bad outcome not yet you trusted me once trust me again we have to act fast
23:37okay i will trust you mostly because i don't have any other option
23:57all right her bp is settled down what do you think you're doing joe clearly doesn't want me in the room
24:03neither do you so i thought i'd help with an x-fix in or3 well you thought wrong what the hell am i
24:09supposed to think then ever since we found out it was twins joe has she shut me out and i'm usually
24:17pretty good at carrying on but today i can't come here when i was in labor with my son his father was
24:33getting emergent brain surgery now i initially didn't know about it because my ob never let it
24:40show on her face she knew that i was at my most vulnerable and she supported me with a strength
24:49that i will never forget joe has to do that on a daily basis she treats those at their most vulnerable
24:59and stays calm right they could be bleeding or have a prolapsed cord she stays strong and encouraging
25:08because that's her job but today it should be yours she's terrified so pull yourself together so she can fall
25:20apart the essential lines back in there's no signs of bleeding or expanding hematoma well there's
25:34some bleeding from the intercostals probably from the fall 20 silk i gave you specific instructions to
25:41strap the patient in on the table let me finish your mistake was extremely dangerous and completely
25:49unacceptable we give our patients the very best care that we are capable of giving no matter who
25:54the patient is or what he has done and if we can't then we leave before our personal distractions kill
26:01someone i should kick you out of this or but instead i'm going to make you stand there
26:05and watch these repairs which shouldn't even be happening in the first place am i clear
26:19hey hey yeah once the temperature started to drop people started leaving sure do you know where the
26:26fans go i don't work too longer than me well uh so you don't know where they keep the extra ice packs
26:32just pack it up you know someone will figure it out oh don't ask or it'll be you
26:36i hope there's not that i need to donate time so i was probably just wishful thinking at this point
26:49adams adams okay okay all right come here come here come here just take a seat
26:57take a seat
27:02hey feel the ground under your feet do you feel the ground under your feet good good
27:07now just keep looking at me keep looking at me right here
27:12you're in the clinic you're not alone and you're safe you are safe
27:18let's breathe
27:39i just came down to say a prayer and i can't get the words out may i
27:46you know jackie's always been so full of life and seeing her in that bed just it breaks my heart
28:08she's strong and resilient whatever happens she'll keep kicking ass and taking names there's no
28:16doubt in my mind
28:21i'm not a very observant jew but i've always been so moved watching people stand during services and
28:29give a misha barack for their loved ones
28:35it's the prayer for healing it's a mouthful i know it sounds beautiful
28:46it's a mouthful i can't feel a thing so no we're hoping to change that all right i'm going to insert the drain
29:15my mom's scared that this is going to make things worse she tries to act all cool around me but deep
29:20down i know she worries are you scared not really i was scared at first when i got diagnosed but now
29:28i don't know i feel like if i think too much about it i'll be too scared to do anything kind of like
