29:05the parent of the year there's just no possible way to give equal attention to six kids we owe it to
29:11the kids to give them a good experience and this is a test for balen can she look beyond what's
29:18happened in the past she's come far enough and she's she's getting further where we can at least

34:02my name is dana wachter and i'm a registered nurse certified in cbit therapy cbit is an acronym for
34:08comprehensive behavioral intervention for ticks so i help those with tourette syndrome and tick
34:13disorders learn strategies to manage their tics
34:24dana first came into our life when it was such a dark spot for balen and it was a dark spot for us
34:29you watch your kids suffer feeling lost like in the dark helpless even after you get the diagnosis then
34:36you come back here and you're like all right now what do we do so there's lots of medications and
34:42therapies to explore and you just start at the beginning and see what works
34:46in my opinion balen has one of the more severe cases of tourette syndrome hey motherfuckers
34:57that's that's the one thing that i'm so embarrassed about is my corpulalia like i'm embarrassed by it
35:03to have ticks where which we call coprolalia where you have the urges to say i've seen things that's
35:09only seen in 10 of the cases we'll work on the coprolalia today okay okay that's a good one
35:20cbit therapy is not a cure it's just a way for individuals to manage their tics more effectively
35:26your goal here is to decrease your tick severity you want to live on your own there's some concern
35:33there so what i want to do is number one first to see if you're aware when you're about to say an
35:41obscene word so while we continue speaking if you feel the urge to say something obscene just place
35:47your finger up so i know that you're and not say it no say it but just go ahead dick okay so you felt
35:55the urge come on okay great now every time you say an obscenity dick okay tell me what the urge
36:02feels like it feels like my skin is crawling okay as we continue speaking whenever you have the urge
36:09to say an obscenity put your finger up and if you just breathe through the urge that sensation will
36:15go away it's just hard to like get there so now when you have that urge to say an obscene word
36:24instead of saying it what i want you to do what it up is breathe in through your mouth
36:29it's reverse breathing breathe in through your mouth and out through your nose you're going to take
36:35a breath in for four inhale two three four breathe out through your nose for six exhale two three four
36:44five six and you're going to do that about seven times which is a minute
36:51now if the urge is gone put your finger up okay so now it was gone at four good excellent so it took
36:59four breaths for that urge to go away if you still feel the urge keep breathing until the sensation goes
37:06away you're now retraining your brain to say i don't have to tick that is a practice whenever you
37:11feel a vocal tick coming on you can stop and do your breathing okay so my hopes for balen is to
37:19get her tick severity down to a point where she can comfortably live on her own where they are mild
37:26to possibly no tick which might be very challenging because at her age um and her severity that might
37:34not be the case but i definitely am hopeful that we can get an improvement where she can live on our
37:39own i want to have a family one day i want to drive and i i mean i don't know if i'm ready to use a
37:46stove yet personally um hopefully we can get you there yes i would love to and i'd love to be able to
37:52use the oven yeah yeah so um i think that would be the next step the oven no well definitely make
38:02that a goal i'm like there's an open right in there getting you to the point where you can use that oven
38:07where you can live on your own like any other therapy it takes a lot of work i'm working i'm not
