00:00Hello there, welcome to another episode of putting Dr. G on the spot in the month of
00:04March and when we are focusing on various conditions that is causing the ejaculate disorder
00:11and we're having second part of talking about premature ejaculation.
00:20As you know, we previously highlighted that International Society of Sexual Medicine indicated
00:27that premature ejaculation men ejaculate less than 1 minute or so.
00:32In fact, there are many other studies highlighted that this is quite common.
00:36The most recent one is Asia-Pacific PEPA study, PEPA stands for Premature Ejaculation Prevalence
00:44and Attitude Study, highlighting that it can be as high as 30%.
00:48Therefore, 1 in 3 men might have this problem and that also includes several populations
00:55that we recruited from Malaysia.
00:59So what exactly causes and what are the treatments?
01:02In fact, what is highlighted that men with premature ejaculation probably have two problems.
01:08Number one, in the brain, the level of serotonin or 5-HT is perhaps slightly lower and number
01:15two, it might be because of the sensitivity in the external genitalia and therefore resulting
01:22in very quick sensation stimulation.
01:26So based on those two things, can we solve this problem?
01:29Yes, there are two types of medication that can do it.
01:33One type obviously is to numb the penis itself but many couples don't like that.
01:39For example, using something called SS cream or lignocaine gel, it will numb the penis
01:43and some men will find that the whole sensation is completely pointless because there's no
01:48point having no stimulation during sexual activity.
01:52Therefore, there's an introduction of a second type of medication which is called SSRI or
02:00Serotonin Reuptake Inhibitors.
02:04SSRI actually is a drug that actually will push up the level of serotonin and therefore
02:10enhance the level of interval of ejaculation.
02:15In fact, it's well-known that SSRI such as Dupoxetine can increase the level of duration
02:21by four-folds.
02:22Therefore, for somebody who ejaculates in one minute, might prolong it to about four
02:27minutes.
02:28So therefore, with such licensed medication, it can change many people's lives and couple's
02:34functions or performance between the sheets.
02:38As we move away from premature ejaculation, we want to talk about a condition called retrograde
02:43ejaculation.
02:44Retrograde actually means backward flow of ejaculation.
02:48Can that even happen?
02:50And what exactly happens during retrograde ejaculation and will it cause any harm to
02:55your health?
