At just 22 years of age, Hafiz Azman was widely considered to be one of the brightest Malaysian riders. In 2018 he contested the Petronas AAM (Automobile Association of Malaysia) Malaysian Cub Prix Championship, riding the Yamaha Y15ZR #30 for team Petronas Sprinta Yamaha-CKJ Racing, having previously raced in other categories in the Malaysian Motorcycle Championship as well as in the Asia Dream Cup. His father, Nor Azman Mad Darus, was a former motorcycle racer.

About 16h00 on Sunday, 16 December 2018, Hafiz Azman was taking part in the 20-lap CP150 Final Race on the street course of Dataran Bandar Penawar, near Johor in Kota Tinggi District, Malaysia. The event was part of the program of the final round of the series.

On the last lap of the race, he was running in fourth position. Heading into a chicane, the #50 bike of Ahmad Afif Amran, who was running in front of him, struck a board at the right side of the track and crashed on the other side of the track, sending its rider to the ground. Hafiz Azman was unable to avoid impact and ran over the fallen rider. He crashed heavily, hitting with his head on the asphalt and then on the fence beside the track. He was taken unconscious to Hospital Bukit Tinggi in Klang, Selangor, with serious injuries from which he died some hours later.

After the tragedy the race was immediately stopped with red flag. This was reported to be the second fatality involving a Cub Prix rider in 18 months; on 05 May 2017 Norizman Ismail was killed at the Bandar Baru Kijal race track in Kemaman district, Terengganu, Malaysia.
